Jamie received his undergraduate and dental degrees from the University of Michigan and his master’s degree in orthodontics from the University of Detroit-Mercy. He is a Diplomate of the American Board of Orthodontics, Member of American Dental Association and the American Association of Orthodontists, and travels across the country and around the world for continuing education to stay abreast of the latest orthodontic technology and trends.
Jamie lives in the Detroit area with his wife, Dawn, and their three children and can frequently be found on a beach volleyball court or on a wake board, when not spending time with his family.
Jeff is an internationally sought-after lecturer renowned for his unique perspective on quality orthodontic treatment results and clinical efficiency. He has presented all over the world, from Russia to Europe, on topics including Clinical Efficiency, Digital Orthodontics, Efficient Early Treatment, Orthodontic Treatment Mechanics and Treatment Planning for Facial Esthetics.
He has been published in numerous journals including the Journal of Clinical Orthodontics, Seminars in Orthodontics, Clinical Impressions and The Progressive Orthodontist. Jeff is also a co-founder of OrthoFi which has dedicated itself to making cutting-edge orthodontic treatments more affordable for consumers.
Jeff graduated with a BS in Economics from Syracuse University prior to receiving his DDS and Orthodontic Specialty Certificate from the State University of New York at Buffalo. He is a member of the American Association of Orthodontists, the Northeast Society of orthodontics, and the Connecticut State Dental Society.
He has been practicing orthodontics for more than 20 years and resides in Waterford, CT with his wife Amy, a pediatric dentist, and their two children. A former competitive endurance athlete, he has completed 5 Ironman Triathlons and the grueling Mt Washington Bicycle Hill Climb more than 20 times. While his competitive triathlon days are behind him, Dr. Kozlowski is still an avid cyclist and spends most of his free time outdoors skiing, hunting, fishing, spearfishing, and wake surfing.
Chip Hurlburt is CEO of Orthodontic Partners. Most recently Chip served as CEO and President of National Heritage Academies which has grown to be one of nation’s largest for-profit operators of K-8 charter schools in the United States. Chip spent the prior 16 years in a number of leadership roles for Kaplan, Inc the global educational services company, including divisional president for two different fast-growth businesses. During this time, Chip also was instrumental in building Kaplan’s healthcare education and training business for doctors, nurses and dentists.
Earlier in his career Chip served as director of marketing & communications for CGFNS International, an organization responsible for screening foreign healthcare professionals seeking to practice in the United States.
Chip holds a Bachelor of Science and MBA from Drexel University in Philadelphia. He resides in Michigan with his family where he enjoys cycling, skiing and tennis.
